Certificate issuance

TYPES OF CERTIFICATES

Enrollment Certificate 3-7 days

Degree (Master's) For those who took a master's degree at FBS
Withdrawal Certificate with Master’s Degree (Expected) For those expected to complete a master's course
Doctoral Completion Certificate (Expected) After applying for your doctor’s degree
Withdrawal Certificate with Required Credits (Expected) Apply only in the last academic (5th) year
Withdrawal Certificate (Expected) After submitting a Petition of withdrawal
Certificate as a MEXT Student 1 week see "MEXT certificate"
If you lose your Student ID, obtain a new one at the Student Center

Certificates noted as "ACIM" can be issued by the Automatic Certificate Issuing Machine.

<SUITA> Suita Student Center, Medical School 1st floor entrance, School of Engineering U1M

<TOYONAKA> Toyonaka Student Center, Liberal Arts and Sciences Organization, Building A 2nd Floor lobby

Other certificates are issued by FBS Educational Affairs. Please apply using the procedures below.

HOW TO APPLY FOR A CERTIFICATE

Direct Applications for Certificates:

  • *If you live far from FBS, an application request can be sent via email. In this case, the email must be sent via OUmail to verify your identity.

Applications via mail:

  1. an Application for Certificate (Word)
  2. a photocopy of your personal ID (Student ID, Driver's License, Passport, etc.)
  3. a self-addressed stamped envelope (SASE)

PROXY APPLICATION

  • If you cannot order a certificate due to situational circumstances, please fill out a proxy application.
  • Send us the documents below:
  1. an Application for Certificate (Word)
  2. a photocopy of your personal ID (Student ID, Driver's License, Passport, etc.)
  3. a self-addressed stamped envelope (SASE)
  4. a Letter of Proxy
  5. a photocopy of the proxy's personal ID (Student ID, Driver's License, Passport, etc.)

MEXT Scholarship Student Certificate

Fill out and submit the form, "Application as a MEXT Student" to FBS Educational Affairs.
