TYPES OF CERTIFICATES

- If you lose your Student ID, obtain a new one at the Student Center
HOW TO APPLY FOR A CERTIFICATE
Direct Applications for Certificates:
- Submit an Application for Certificate (Word) to FBS Educational Affairs Section via email. The email must be sent via OUmail to verify your identity

PROXY APPLICATION
- If you cannot order a certificate due to situational circumstances, please fill out a proxy application.
- Send us the documents below:
- a photocopy of your personal ID (Student ID, Driver's License, Passport, etc.)
- a self-addressed stamped envelope (SASE)
- a Letter of Proxy
- a photocopy of the proxy's personal ID (Student ID, Driver's License, Passport, etc.)
MEXT Scholarship Student Certificate
Fill out and submit the form, "Application as a MEXT Student" to FBS Educational Affairs.
